Health Department Permits - Info & Downloads

I just got an email informing me that I used the wrong form for submitting my permit request. It was the same form that I used in previous years, so I'm not sure when this changed. However, so that others don't make this same mistake, I've posted links both to the correct form, the main page, and various other special links on that page directed at the BM community. (They were very nice and said that they would process the permit request anyway, but would prefer we use the right form next time.)
