I am bringing an 8 1/2 X 11 document that has the following information on it.

My Name
Where I live
Emergency Contact Information
Blood Type
Medical conditions I live with
Medical Insurer Information
Primary Care Physician's Name
A list of Current Medications I am taking

I am taking this information for use by on-site medical staff in the un-likely event something were to happen to me.

I believe having this type of thing whether typed up nicely or handwritten on toilet paper would be prudent for those of us who have these issues in our lives.

Burning Man staff...Perhaps this should be an item in future Survival Guide Issues?? Kinda goes along with the "Radical Self Reliance" thing.

You can always do an index card and use poor man's lamination (clear tape)

Also include allergies to meds, and date of birth.

but seriously, you have a good list, we medics love these. you can seal it with the laminations stuff found at most office supply stores.
I get to drive the ambulance how fast?!!
