And the real "cost" is the discounted price that the denial industry actually pays, not the inflated retail charge that is billed to individuals.

That "no value added tax" cost added by the health care denial industry is what is used to pay for universal medical care in canada, and other places.
The current rate of profit and administrative cost increases is unsustainable, as the rate of bankruptcy by otherwise solvent people, due to medical bills, shows.
It currently requires something like 16 clerical workers per provider, in this market, just to keep the money coming in. Unless you severely limit your insurance participation.

The insurance companies employ many more people than that, of course, and are raking in huge profits. Plenty of money left over to fund political action committees, unfortunately.

Seriously nobody is going to stand on the side of the road talking about payment while the patient is bleeding out. EMS is hard enough as it is, dude, don't ask them to get into all that bullshit.

Might as well ask firefighters to stand and watch while a house burns down, if the landlord hasn't paid the fee. Oh, wait...
Yes, the firefighter thing has happened, as hard as that is to believe. Seat belts wouldn't be that hard. Give emergency care, they get to the hospital and no means of payment? Sorry.
I'm goin' with JK here (High 4). and same with helmet laws..sure, don't wear it, and, yeah, I suppose you gotta take someone bleeding to ER. But, once patched, no public monies.
find your own rehab, live with the big scars, no disability, either.

Let us know how you decide when that's cost effective.

We already do many of these things.
When the people involved no longer work or pay taxes, what is the cost?
What about when their job was a vital one?

How about the family that quit jobs to be caretakers?

These things already happen.

I read a report that mexico has started decriminalization, in desperation.

Last count I saw was 35,000 dead.
I presume this to be low.

Mexico now has relocation camps for refugees from the drug war.
Is that a first?

Sure, that's how it normally works. However, what if you live in the boondocks that aren't incorporated into a coherent township that has fire service? I suspect that is the kind of area Cranick lived in, especially considering that he was burning his trash rather than having it picked up. Does that then mean that there is no service? Or if there is service, does that mean that larger municipalities have taxation jurisdiction over unincorporated satellite communities? If no such jurisdiction, are the larger municipalities required to provide service anyway?

If there is no infrastructure for fire services because of how rural the area is, I don't think then it would be expected that fire service would be provided. It would be a risk a person that decides to live/build there should be aware of.

My argument would not hold for that situation.

Interesting and true. I did a search for that patent number on the official US patent office site and it was there as well. Knowledge is power.

So now we know why the enforcement of the War on Drugs is so popular with government law enforcement.
