Wasn't that one a standard Butterick pattern in the 50s? Came with a free lobotomy coupon...
Seriously though, it is basically a long-sleeved button-up shirt pattern worn backwards, with way-long closed end sleeves, 3extra-wide front (on original pattern, back in straight jacket version), and assorted buckles & straps (one for each arm, 3-5 across the back, one from front to back over the "groin area") added.
Are you looking for a functional, decorative, or gimmicked version?

M*A*S*H 4207th: An army of fun.
I don't care what the borg says: feather-wearers will NOT be served in Rosie's Bar.
When I ask how many burns, I mean at BRC.

Tom, if you manage to get ahold of a pattern is there any way you could make two? I'll gladly pay for your services as long as it's a little cheaper than store bought. I've always wanted a sssssssssssstraight jacket... I'll work some Google magic and see what I can find, plus I know some costume designers from Tribe.net who may be able to point us in the right direction.

Lonestoner beat me to the punch on a few links but:
This isn't a pattern, but someone posted some pretty good pics of french-style straitjacket. Might be helpful if making your own pattern.

Detailed pics of a variety of straitjacket styles:

Using ductape to make a pattern (kinky straitjacket):

When I was a kid I used to pray every night for a new bicycle.
Then I realized that the Lord doesn't work that way so I stole one and asked Him to forgive me
