Oops. I meant to include a note...
Black goes to the the hot side of 120VAC, White to the neutral, and brown & yellow to the motor. I'm pretty sure the output of the controller is DC so you can swap the brown & yellow with each other to reverse direction.
A cool trick to reverse polarity with a switch for DC loads:

Its a DPDT switch although a relay works too. Relays wired this way are great for automation or remote operation. I did this for my little electric scooter to get reverse. Should've seen the looks on peoples faces while I zipped around in reverse
Crap, tried editing and it won't come out right. input is at the terminals at either end of the switch and cross them to the other end terminals. The output is from the middle common terminals.
