I actually didn't make up or choose mine.
Whenever I used to call my friend Phl that lived a few miles away, his wife usually answered. I used to just say "Goddammit... " and she'd say "What the hell!". It became the standard script. She'd yell across the house "Phil, it's 'goddammit' on the phone!"
Then I built a boat... when you are driving a boat, everyone calls you "Captain".
And so it came to be.

That's what I do. But you can call yourself whatever you want. The classic method for acquiring a playa name is for someone else to name you during something memorable, but there's no reason you can't choose one, or use one you already use.
I'm going to introduce myself at least once this year by Junglesmack's Grandma's cat's name:

If you're in a camp, and your camp communicates via facebook, your playa name will very likely be whatever name that is on your facebook profile, which is most likely to be your default world name - or given name. I don't really care to get into some debate about what is default, etc.

As others have said, use what you like. Some people go by their default/given name. Others are given nicknames by campmates or friends either on or off the playa, and still others pick their own. Whatever works for you and makes you happy, though of course sometimes you go out there with an idea of what you'll be called only to find some completely other thing happens. The playa's funny like that

Seems to me it would be hard to say all the time.
Although, it's written on my arm, so maybe I just have to come up with an introduction where I let people read it and thus I don't have to keep repeating one seven one of three four eight or one hundred seventy one out of three hundred and forty eight or whichever other jumbled iteration comes out of my mouth... Probably some kind of litmus test for how BORG vs. Art I'm feeling at the moment.

Right, we don't need full precision. Since there are only 50k-60k people out there, precision to the hundred-thousandths should be fine. "0.49138" rolls off the tongue pretty well.
