That's nothing new, that's how boat shower systems work (using a heat exchanger to heat water with the engine coolant) although they also have electric heating elements for when you have shore power.
You could buy that system ready-made from a marine supply outfit, or piece it together yourself.

My real boat does, yes. And it uses a standard marine engine heat exchanger, which works very well. You can fire up the motor and let it idle a bit and you've got hot water.
I'd use a camper or boat shower head if I were building that setup, because they have an on/off switch right on the head - great for saving water, or maintaining your temp setting if you have a hot and cold supply.
